Shouldn't need triggering either. Normally the router will allow outoging connections on ports 500 and 4500 (or any port) with no configuration.
If you have other devices on the network using those ports, maybe via UPNP, that can cause issues.
Also, if your Orbi is connected behind another router, or in DMZ mode behind certain routers, then this can block VPN. See this thread:

How is your Orbi connected to the Internet? Is it behind modem or router? What's the make and model?

Both iPhones show they are unable to connect to Qustodio VPN when connected to my Orbi Wifi. If I disconnect them from the WiFi, they are able to connect to Qustodio VPN via cellular data.
I am not aware of other devices on my network using those ports. My Orbi is only connected to the internet by a cable modem [Motorola Surfboard SB6120], there is no other router.

OK, so it's not a phone configuration issue. What IP address is your Orbi getting on the WAN interface? You can post just the first two numbers if you want. Is it a public address, not a private address like 192.168.X.X or 10.0..X.X?
You might need to try a factory reset of the modem and the Orbi.

Under the "Advanced" tab, under "Router Information" it shows IP address 10.0.#.# and under "Internet Port" it shows IP address 68.101.###.###
