Orbi Voice RBS40v cannot register Alexa and shows disconnected on orbilogin.com

Question

have an existing RBR40 setup with 2 RBS40 satellites. V2.5.1.22 firmware.

Added a RBS40V (v2.5.1.6 firmware)

I have removed and reconnected the RBS40v a few times including hard reset.

Removed the amazon app and alexa app on my Samsung S20 and also tried it on a Note 9.

I confirmed I am in "Orbi Voice" mode, white lights not Orbi Extender mode.

The orbi app shows the satellite is connected but when trying to connect to alexa, it gets stuck on the Amazon Alexa Language screen and errors out.

On Orblogin.com the connection satellines shows the 2 RBS40 satellites but the RBS40V is listed but shows a blank IP address, blank connection type, and is shown as disconnected.

I am not sure what else to do.
