Your cut-n-paste is missing the last line of the WWAN output, titled "Modem software: Unknown" as it did for me.

For those who know, the driver in question is downloaded from NetGear and is called AirWatch v2.5.0.
In my case, I am using AT&T mobile broadband provider, and use the following driver from: https://www.att.com/devicehowto/tutorial.html#!/stepbystep/id/stepbystep_KB421838?make=ATT&model=BeamAC340U
