Sim not provisioned

What's it mean when an error says sim not provisioned?

I would expect this means you're carrier did not activate/provision the SIM.  Is this postpaid or pre-paid?  What carrier?  Is your Nighthawk unlocked / same carrier as SIM?

This was an exchange with ATT as battery failed on old unit.  Ended up keeping old unit and replacing the battery only.  Suspect it is a SIM that was not programmed by ATT.
