Are you using IPv6? If so, make sure to set that up as well. 

I use a similar setup with Pihole and it works fantastic. With that said, its through an orbi and not a LAX. 

Without access to an LAX to play with, tougher for me to troubleshoot.

      I don't have an LAX but is there any settings for the SIM card setup that impact DNS?

        and on reboot,is it retaining your DNS settings?
