Can't answer the complete question, except of this:
Simultaneous bridge and client association is intentional - in fact it's a feature, certainly on the WN604. The MAC addresses are purely for the point-to-point resp. point-to-multipoint WDS connections (max four), and don't limit the AP functionality. To configure more access controls for the wireless AP mode, head to Configuration > Security > Advanced > MAC Authentication.
