What is the Mfr and model# of the ISP modem the NG router is connected too?

I currently have anti-bufferbloat on never and I have my Xbox prioritized in QoS but to no avail. My xbox upload speed is under 1.0...My voice is garbled in xbox live parties but I can hear everyone else fine. I am beyond stressed and just want my speeds back at what we are paying for which is about 25 for upload and about 200 for download. We already hard reboots, checked for updates, on modem and router.
Please help.
