It's probable you have Private Mac Addresses enabled for the WiFi on your phone (it's enabled by default), the router will read this as a different device every time the MAC address changes. I would recommend disabling that, then check the IP the phone has on the phone itself then look through the devices on the Device Manager on the router to find the IP that matches. That will be the device and you can rename it. As you'll have disabled Private Mac Address you shouldn't have the issue again.
