Try putting the IP Phone in the RAX router DMZ.
Also the WAN ports in back of the CM1200 modem are only assigned by the ISP and most ISPs only assign 1 port in back for a WAN IP address, all other ports are not usable, unless the ISP supports additional WAN IP address assignments. Something to ask your ISP about.
