SIP ALG has been known to cause problems with VOIP phones. By default it is enabled on Netgear routers. Go to Advanced -> Setup -> WAN Setup, toward the bottom you should see a checkbox to disable SIP ALG. You may have to reboot your router.
SIP ALG is a good start point for a web search regarding configuration of routers to support VOIP phones.
