Can’t get netgear_ext to come on my iPhone to set up

Did you set it up using wps?
If so, its default setup is mesh. meaning it uses the same ssid as the router.
if you haven't done a thing yet, then try factory resetting it. push and hold reset for 7-15 seconds while its powered on.
