Yes.
And while you're at it, reserve an IP for the extender so you always get the same one whenever it makes a connection.

I made both of those changes but that has not helped. Could my ISP be blocking it in any way?

I'm betting its more something on verizon's end. If its connected to the router and the router is giving it an IP address.
So its connected, its just not working properly.

What brand and model number of cell phone are you trying to use to connect to the LTE extender?
Does the phone properly connect to the Verizon data network at other locations?
Does the phone not connect to the extender for phone calls or cell data or both?

My phone is a Samsung Galaxy S9+ and was connecting fine before. The problem is not that my phone won't connect to the extender. The extender is not connecting to the Verizon network. Verizon says the extender is working properly and that I should contact my ISP. I don't see why my ISP could make a difference.

You're kind of caught in limbo of is it the router or is it the extender.
So cheat. Disconnect the router from the modem and power the modem and the extender off for a couple minutes.
power the modem on and give it a couple minutes.
Then connect the extender directly to the modem and power it on.
Give it a couple minutes.
IS it working? If so, it still can be something in the router.
No, then it implies the extender is the issue.

I will give that a try.
