I asked because Netgear has been blocking "remote access" as a security measure.
Perhaps I should have asked about this:
SMB server which is wired to the router, and a desktop connected wirelessly.
You are playing with various things that need to cooperate. In this case, both the server and the desktops need to play nicely together. The router is just one link in the chain.
People need to know as much as possible about what is happening at your end before they can begin to ponder on what might be going on.

it's a straight forward setup. Nothing special. We access the server in the office fine. DMZ is setup on the RAX49, and it all works. For some reason, only the port for SMB (445) doesn't get to the server from the outside. The RAX49 log doesn't show port 445 coming through either. I have tried multiple computers, and I get the same results. a timeout on the computer. I'm thinking.. the issue belongs to the Internet provider. Maybe they are blocking the port on their gateway, BUT they same no. Wish there was a way I could do a traceroute on that port and see where it stops. I tried. BUT I will continue to plug away at it. THANKS for the reply. Will post the out come when I can,
