What actual phone is it?

I'm not sure on the modem, I do know the ISP is OmniTel Communications.
The phone is a Yealink T46S, the phone company is Momentum Telecom.

MIght find out what their home gateway/ont is. That can be causing issues too.
Do they have ipv6 enabled?
Also, they can try setting the phone with a reserved IP address in the router and then setting that address in the netgears DMZ

The modem is a Cyberlink, I do not have the model.
IPv6 is disabled on the router.
I'll talk to the phone company today about using the DMZ.
