As part of the Trouble-shooting I have updated the Firmware of the XAVN2001, and followed this by a factory reset. The assigned SSID is seen by the S4, and then I manually enter the passcode. This seems to be accepted, as it does not generate an error message about password, and it then tries to Acquire an IP. It is at this stage where it fails.
I have been working with Netgear on a ticket, and they have tested it on a GSM Galaxy S4, where it apparently works, but they said they don't have a Verizon S4 to test it with. I am still waiting to hear back from them after my latest report to them.
For what it's worth, here are 3 screen captures showing what I see on my phone as it attempts to connect ("Bedroom" is the SSID for the XVN2001):

As I've said before, I have no problems connection with multiple other devices:
Verizon Samsung Galaxy Nexus
Verizon Motorola Droid 3
Verizon Motorola Razr HD
Asus Infinity Tablet TF700T
Very frustrating.
