EX6120 constantly losing connection

The extender is only a couple of weeks old and continually drops the connection between it and the device.  This happens regularly - the signal returns usually within 10 mins, but then drops again a bit later.

I have tried a factory reset but no luck.  I also tried logging into the extender using but I cannot seem to gain access at all.

 

help much appreciated.
