RAX45 Nighthawk will not save port forwarding

I purchased the RAX45 from Costco in February of this year and it has been performing admirably until today.

 

I have (had) one port forward to my Blue Iris server.  Today, I could not log in on the port forward.  When I checked the setup on the modem, the port forward was blank/gone.   

 

When I try to re-enter the port forward and "Apply" the setting, I am returned to screen which shows no port forwards.  Nothing has been saved.

Any ideas greatly appreciated!
