1. what modem/gateway is it connected to?
2. what firmware is on the router? (please don't say the latest)
3. when it happens, are the devices saying "connected, no internet", are they disconnecting from wifi entirely, what is actually happening?

It is a Netgear modem CM1100 - the modem is working just fine.
Firmware is 1.0.9.88 - I know this is not the latest, but Nighthawk app says no newer available.
Devices say connected, no internet.
I can always log into the router via anywhere access in the app, even when my wife at home says it is not working while I am at work, so I know it is not the modem. The router worked flawlessly for about 3 months, then this issue started happening about once a week. Now it is every couple hours.

You can manually update to the 1.0.11.123 version thats out.

then try a factory reset and reinstall

Thanks for your help! Forgive my ignorance, but is there a way to update the firmware without a working computer with an ethernet port? My wife's laptop doesn't have one, and the motherboard went out on mine recently. Or will it work to update to 1.0.11.123 via the routerlogin page?

You can upgrade over wireless. We recommend doing it hardwired just because when we see corrupt firmware issues, it tends to happen when people upgrade over the app or over wireless.
But many doing it over wireless without issues.
