Does it work if you assign the same interface ID that you are getting from your ISP? Though this can be tricky, since you might get a new IID when the lease expires.

ISP doesn't assign the interface ID on the LAN side, it only provides the /64 prefix for the router LAN. For now the issue seems to be resolved with a reboot. I am setting up my own interface ID rather than using the router generated interface ID from it's mac address for simplicity and reaching the router ipv6 address locally.
