I had this same problem and i was trying to reload software on multiple usb drives and nothing was working. Then while searching on the netgear community for a solution, someone mentioned to run the USB software as administrator. So, I plugged in my USB drive right clicked the usb exe file and clicked on run as administrator and reformatted drive. I reinstalled the recovery software on the same usb drive and tried loading from usb and it worked. Im not sure if running as administrator made a difference but as soon as I did that nas was able to recover from usb. Hopefully this works for anyone with the same issues.

I tried 6 different USB's, then I tried running the USBrecovery tool one last time, this time I right-clicked the file and ran the file as administrator. Formatted the USB-drive with the USBrecovery tools built-in format function and then created the boot... Finally worked like a charm!

+1 for USB format... right click the usbrecoverytool.exe and choose "run as administrator" and then the recovery worked first time on RN314, after several failed attempts without doing this. process takes 60 seconds or so.
