You may get some clues from how the add-on does things.
You could look at e.g. /etc/frontview/apache/addons

There was no URL posted. mdgm posted a path on the NAS (e.g., cd /etc/frontview/apache/addons ). The idea was to browse there, and look at how the configuration was set up.
