Sounds like the original NV+ (called a v1 here). Does it say "ReadyNAS NV+" on the front panel? Note if it says "ReadyNAS NV+ v2" it's a different platform (and could accomodate larger disks).
The admin portal should also confirm this - you should see the NAS model in the upper right of most of the screens. The home screen should also have a model field, and also would have the firmware version (should be 4.1.x for an NV+ v1). It'd be useful to know the firmware version.
It would also be useful to know if your admin password changed back to the default when it switched back to X-RAID. (The default for the NV+ is netgear1.)

FWIW, X-RAID is a application layer on top of standard RAID which makes RAID expansion easier to manage - it is actually still running RAID-5.

Can you please download the full log zip file from the admin portal?
Select "status" from the left pane, and the "logs". "Download all logs" is on the upper right. Though the screenshot below is from a different model, the log screen should be the same as what you have.
The system will then create a zip file (which can take a minute or so), and then should download it to your PC/Mac.
You can put this into cloud storage (dropbox, google drive, etc) and share the link, so others can take a look. Don't post the link to the zip file publicly - send it in a private message (PM). There is some privacy leakage. You can send a PM using the envelope icon in the upper right of the forum page.
I'd be happy to take look (though note I don't work for Netgear). If you'd prefer that a Netgear mod review the logs, you can reach out to either Marc_V or JeraldM.

It is a ReadyNAS 104 running 6.9.5.
The Admin passwords have not changed.
I'm happy for you to check the logs, I will send them over to you directly.
