I understand it is frustrating. The fact that the shares are gone is more troubling to me than the FV and RAIDar status (though of course they are likely related).
The direct path is to give up on the data and do a factory reset. Before doing that, perhaps try powering down the NAS and booting up with drive 5 removed, and see if what happens. You can also repeat that experiment with drives 4,3,2,1.
