CM500 Only Displaying One Upstream Channel

CM500 Only Displaying One Upstream Channel
I'm not entirely sure how big of an issue this is, but I restarted by modem two days ago and ever since it has displayed a solid amber upstream light indicating only one upstream channel being locked. Upon doing some troubleshooting, I discover that my modem is now ONLY displaying a single upstream channel and not four channels as it should (screenshot and logs attached). I'm not having any connection issues at this time, however, it seems unusual to me that only one channel is being listed and I'm not sure if I should be concerned about this. Besure there are not coax cable line splitters between the modem and ISP service box.
Use good quality RG6 coax cabling up to the modem.
Contact the ISP and have them check the line signal and quality UP to the modem.
