Connection Issues - Only Upstream Light Off

I have a CM1200. I've been at this house for around a year with no internet issues, but this week it keeps dropping. Unplugging the modem for 30 seconds always fixes it for at least a few hours. I tried a factory reset on the modem and that fixed it for a few days. When the internet drops, all of the lights are on aside from the upstream light. I have the event logs, but I am unsure of how to interpret them. Any advice?

Please post a copy and paste of the modems connection status page. Please use the Choose File button on your next post for this. 
Have the ISP check the signal and line quality UP to the modem. Be sure the ISP provisions the modem correctly.
Be sure there are no coax cable line splitters in the between the modem and ISP service box. 
Be sure your using good quality RG6 coax cable up to the modem.

Sorry -- I tried to add images to the last post but it seems like they didn't go through. I attatched a file with the connection status on the first 3 pages and the logs on the last 3.

The coax cable is fine and there are no splitters. My ISP is not particularly helpful, but I can try to get them to check the line quality on their end. Does it seem like anything else may be going on from what I've attached?
