How do I port forward this modem for UDP to ports 5198 and 5199, and outbound TCP to port 5200 for echolink?

The CM400 is a pure modem. It doesn't have a router. Therefore, port forwarding is not necessary.
If you have a router plugged into the modem, then you would need to set up port forwarding on that router.
