New modem DM111PSPv2 not finding DSL connection

I have Century Link and worked with them and Netgear on this problem, and they both could not give me a solution.  The third light on the modem, connection to DSL, is not establishing.  It will blink but never stay fully lit.  I believe it is the settings in the modem that is not allowing it to establish connection.  Can anyone help me out?

I am also Centurylink in Utah. At both locations I set ADSL settings @

Multiplesing Method - LLC-Based

VPI - 0

VCI - 32

DSL Mode - Auto

Then at one location I set

Transfer Mode - ATM

At the other location settings are for VDSL

Transfer Mode - PTM

AND

VLAN Tag - Enabled with WAN VLAN ID set to 201

My router is a DGND3700v1 so I don't know where all these settings are on your DM111SPv2
