M4100-26G changes VLAN membership after reboot

We have had this switch in place for about a year, and have had some random problems with it.  At first, if there was a power outage, we would lose the configuration on the switch.  We have updated the firmware, and that seems to have helped, but now, the unit will randomly lose connection, rebooting.  We have a Synology with an iSCSI target coming from the switch to a server, and we only use 8 of the ports in the switch.  Have enabled jumbo frames across all devices and as far as I can tell, everything is set up properly.

 

Anyone else experiencing random reboots?

Kindly answer the questions below:

 

a. Are there any changes in the network setup or in the configuration of the M4100-26G switch that might have caused the random lose connection? 

b. Just to isolate the problem, if you will disconnect the M4100-26G switch from the existing network and just leave a PC and Synology NAS connected, will the switch still reboot on its own or lose connection?

c. Just to confirm, is the current firmware of the M4100-26G switch v10.0.2.20?
