Configure what?
The DM111PSP is 11-year old ADSL modem.
If that is what you own, ask CenturyLink. But ADSL died years ago in most markets.
Maybe you need to be a bit clearer about what you own and what you are trying to do.

of service is CenturyLink providing to you? What were you using before?

Depends on where you are. Some locations use ADSL while others use VDSL. They also use pair bonding for some of their higher speeds. From the ADSL that I set up for a friend quite some time ago I used these WAN settings.
Encapsulation - PPPoE
Mode - LLC based
VPI - 0
VCI - 32
Transfer mode - ATM
I did another person who had VDSL
The big difference was the Transfer mode was PTM
and the VLAN id was 201
Centurylink may have other configurations in other areas, but the above worked for the 2 locations I was in. Salt Lake City for the ADSL and St George, Utah for the VDSL. Both were Netgear modem/routers. If you're pair bonded there's no Netgear Modem/router that supports it.
