LM1200 SIM card purchase
I'm stuck at the very beginning. The SIM card. I want an unlimited plan. Do I have to purchase a phone with a SIM in it and pay on a month to month basis. What is the best way to get a SIM card? I'm an iphone user so this isn't something I'm familiar with.
I have the LM1200LTE Modem and the Nighthawk AC1750.

It will depend on where you are and which mobile operator you plan to use.
For example, here in the UK I can get a SIM from EE that I can use in any mobile device I like. Look for "SIM only" deals:
I assume that your LM1200LTE came with no tie to a particular mobile operator.

Most of carriers support data only plan to be used with such type of devices. For LM1200 sold in US, you can get the plan from AT&T, T-Mobile, Verizon and other MVNOs. When using MNVOs, you may need to manually configure the APN on the device.
