Getting the most out of my xr500 for the ps4 pro

Hello, I have the cm1000 modem with the xr500 gaming router. Trying to get the best set up for ps4 gamming. I run hard wired. But I am still getting into some lag issues. I am not playing on a computer monitor I am on a 55 inch lg TV that's had input lag of 12ms. I have always gamed on a 55 inch TV and I started really noticing some lag about 5 months ago. I have 400mb down 40 upload.
My problem is I dont even come around a corner in COD or Destiny and I am being killed before anyone is there. In COD I play hardcore so it's not that they see me on the mini map and pre fire me. And not only that just about every game it happens.
So i bout this set up to have some of the latest and greatest and still having some of these issues and it sucks.
Some nice configurations for the ps4 pro would be nice. Thank you

For the router, could you follow both of these guides for Geo-Filter and QoS. That'll get everything setup optimally.

You might then want to go one step further, and use Ping Assist as a Ping Filter. This will guarantee the host of your game is below your specified ping. See the bottom half of this guide: http://support.netduma.com/en/support/solutions/articles/16000087579--dumaos-optimal-settings-guide-...

Finally, have you set the TV to gaming mode? If it's not in gaming mode then the input lag could be unplayable.
