No 5G channels on R8000

This is a 3 channel router with 2.4G channel and 2 5G channels. This all used to work fine, but now I can only get the 2.4G channel. I have reset, rebooted, and checked the firmware. This is unacceptable since I have 9 devices so if they are all on the single 2.4G channel.

Smart connect combines the 3 ssid's into a single ssid. It appears as if only 1 band is broadcasting but all of them are when using smart connect. It steers devices between bands as needed based on distance/bandwidth needs.
If you want to test it, disabled smart connect and name each band something different. Then do a scan.

Are you using smart connect?
What firmware are you on?
What are the led's on the router showing? (all of them, color, blinking/solid)

I am using smart connect.
Firmware is: V1.0.4.28_10.1.54.
Top five lights are flickering white. Bottom 2 arrow lights are steady white.
