Phone ports not working

Every other light except Phone 1 and Phone 2 are lit.

Who's your isp?
Have you contacted them? Its their provisioning that allows those to function

You are asking about features specific to the Voice bit of a cable modem/router. People who own those may not hang around in this section for Nighthawk WiFi Routers
