1. No - If no problems are seen after the update. See #2

2. No - Sometimes just a full ON power OFF of the RBR and RBS and ISP modem is only needed. I have been doing this for past few versions of FW and haven't had any ill effects before and after this. I call this a "dirty FW update".

3. Yes - Maybe needed as last resort if #2 doesn't work or help to resolve problems.

3. Yes - For users who like to start fresh after each update. I call this a "clean FW Update". Historically for me,  I have always done this as from personal experience, i've seen odd issues appear after a FW update. NG and NON NG routers.

My Setup (Cable 1Gbps/50Mbps)>CAX80 v2.1.2.1(LAG Disabled)> RBK953(v6.0.3.68) & XR450(v130)
Additional NG HW: C7800/CM1100/CM1200CM2000, Orbi CBK40, CBR750, RBK853 v4.6.3.16(AP) & RBK752 v4.6.5.14, RBK50(v22), SXR30(v110), R7000(v34), R7800(v84), R7960P(v82), EX7500/EX7700, and WNHDE111
