Can someone tell me how to find a return form which was supposed to be attached to an email.

Question

I need to return an Orbi satellite. A form to send with it was supposed to be attached to an email, but wasnt, the form is to be sent back with the defective satellite. Also where do I send the satellite to please? Hope someone can help, I'm tearing my hair out! I live in New Zealand.
