MR5100 and PS5

I have an MR5100 connecting to a PS5 and trying to get rid of the NAT 3 and change to a NAT 2. It is an ATT Wireless hotspot that I use for home internet. I see all these suggestions about port forwarding and such with a bunch of numbers to put in. So this is what I have on my router and I have no idea what numbers to put where. The IP Address is what I am assuming is the PS5 IPv4 address and there I think I have tried everything. Help???!!! The from and the to confuses me and it seems when I finally get numbers to stick it won't connect to the internet. Anyone else have this issue and a fix?  Thanks in advance.
