C3700 modem/router trouble using voip

Guys i really need help. I have a c3700 netgear modem/router . The past few days my voip phone goes from having dial tone to no tone. I have tried restting everything but no luck. My voip provider did a check and said it was  being blocked.Can anyone help me through this..thanks

    Does any ports need forward for the voip service you have?
