Forgot User Name and Password.
Forgot to tell us what you have tried to fix it. Did you try "password recovery"?
If you did not set up password recovery you have one option:
How do I perform a factory reset on my NETGEAR router? | Answer | NETGEAR Support
Then set up the modem/router from scratch and make a note of the new password that you set up.
