The CM1200 modem is not a router. The 4 ports in back are for ISP WAN IP address assignments only. Most ISPs only assign 1 IP address at a time to a WAN Port on the modem.
You need a external wifi router to connect to the CM modem...Then you can have lots of devices connected to the router.

I have my router, a nighthawk AC1900 wire connected to the modem through 1 of the 4 ports on the back of the modem, I have 2 TVs also wire connected to the back of the modem. The router then provides the WiFi signal throughout the house. It has been set up this way for the last 12 months and worked just fine. I wake up this morning and no WiFi and no tv.

who's your isp? Some isp's support multiple public ip addresses.
Yours might have done that and then stopped that option.
Try disconnecting everything from the modem and power it off for 2 minutes.
Then power it back on. After its been on and fully booted for 2 minutes, plugged the router into port 1 on the modem and power the router on.
See if it'll connect/work.
