I forgot to mention we have had this modem for a little over a year and this issue is recent so I don't know if all your solutions are applicable. But I will still try them.
You have correct and uncorrectables. These should be zeros.
Something the ISP should help you look into. Signal on the line. Cable and check connectors as well.
