You have errors in your line and your logs are indicating issues.
check your line for kinks, damage, old/bad/cheap/corroded/damaged connectors and replace them if you have them.
Remove any splitters, amplifiers, or signal attenuators from the line.
also, you can move the modem to right where the coax comes into the home to prevent the line in the home from being the issues.
If you still get the errors, then the ISP would need to check the line.
