Besure there are not coax cable line splitters between the modem and ISP service box.
Use good quality RG6 coax cabling up to the modem.
Contact the ISP and have them check the line signal and quality UP to the modem.

Furry, are you saying I should make sure I have a direct connection from the modem to the ISP without any splitters, correct?
It's odd that I'm getting 400 mbps download and 5 mbps upload. If the issue that you said was the problem, shouldn't my download speed be much closer to my upload speed? Thank you for your help

Yes, splitters can induce noise or attenuate the signal on the line going to the modem.
Someting to get in contact with your ISP and have them help you ensure the signal is good up the the modem.
ALso be sure you have a supporting PC or mobile device that supports testing of 1000Mbps. Not all PCs can perform this test. Some older single or dual core CPUs are not fast enough. Mobile devices may not be spec'd or designed for this as well. Most newer generation phones should be. Contact the mfr to see.
