my modem is acting up please help!

my internet keeps cutting out, and I checked the log this is what I saw, can anyone help? is it the ISP problem or my modem problem?

Have the ISP check the signal and line quality up to the modem.

Be sure your using good quality RG6 coax cable up to the modem.
