Need Help with Sprint NetGear 6100D router with a static IP address assigned to it.

I have a Sprint NetGear 6100D router with a static IP address assigned to it.   I am able to successfully set port forwarding for the first three input slots- but when I get to the fourth port forw...

Are you using the address reservation when putting in the Ip address that you want to forward?
