What Firmware version is currently loaded?
Please post a copy and paste of the modems connection status and event log page.

Be sure your using a good quality LAN cable between the modem and router. CAT6A STP is recommended. 

Does the CM modem with with a direct connected PC or some other wifi router? 

Are there any indications on the router on it's Internet port when this happens? Check it's user manual for LED indications on the routers LEDs.
