You've got errors in your line, your power levels are all over, and only 1 locked upstream.
Check your coax line for kinks, spliters, amplifiers, or signal attenuators. Remove them if you have them.
Check the line for old/bad/cheap/corroded/loose connections. Preferably, move the modem to right where the coax enters the home to prevent wiring in the home from being the issue. Then connect it and try it. if its still having issues, take the same screensnips and post them here. The isp might need to check the line in that case.

The connection from the pole to the house is exposed, that seems like it would effect the signal.
