Should only need to disable SIP ALG if thats whats causing the problem and not DoS and port scan feature.
Using most current released FW version?

Yes, the update firmware check shows no newer version. I'll try re-enabling the Dos and port scan feature, just not this evening; too frustrated to try and then have the wifi calling not work again. Have any ideas why SIP ALG might break the wifi calling? Especially since it seems to be a known issue for some years now, one might think someone would have fixed it.

SIP ALG is a different developer that NG uses. I know for a fact that it works enabled with other model NG products. Possible that it hasn't been fully updated on the CAX80 yet.
Another thing to try is to disable any MAC address randomizers on the phones.

Well, considering I've not heard of MAC address randomizers previously, I'm sure none are on my personal phone, and the work phone is pretty locked down; only apps from the company are available and it has a program called MobiControl which basically disables anything Samsung has on the phones.
The only reason I had any idea about SIP ALG, IPSec ALG or the DoS and port scan functions potentially being the problem was from doing a general search about it, nothing on the Netgear site, and before the call got dropped when the rep asked for the model # he had zero idea either.
Not the first time I've had an issue with something technological and in attempting to find information found that there was a known issue by the general public sometimes for years that tech support for those providers had no idea about, allegedly.

Checked out the link you provided. On my S21 Ultra the only thing that comes close is in the enabled developer options called "Wi-fi non=persistent MAC randomization" which is off. One UI 6.1, Android 14. Nothing under the regular settings that would achieve this. Good info to have, though.
