I personally would not recommend using DMZ for any game console. Could lead to nefarious attackers trying to gain access or do nefarious things while you game online.
What Firmware version is currently loaded?
What Cat version is the lan cable between the modem and game console? CAT6 is recommended.
If you set up port fowarding, then uPnP should be disabled. If your the only one gaming online with xbox, then PF is not needed and uPnP should work for gaming online.
Be sure there are no coax cable line splitters in the between the modem and ISP service box.
Be sure your using good quality RG6 coax cable up to the modem.
Be sure to power OFF the modem for 1 minute then back ON.

Current firmware is V3.01.40, which matches the current firmware for "Other ISP" from Netgear's website.
Lan cable is CAT 5e, so I could look at upgrading that.
There is only one Xbox on the network, and uPnP is on when I started experiencing the issue. When I tried to set up port dorwarding, I did not turn uPnP off, which may have been the reason it didn't work?
There are no coax cable splitters that I am aware of, I don't know a lot about coax cable but mine says it is PPC Perfect Flex 6 series and seems sturdy, and I have tried multiple full restarts (turn off, remove power, wait 1 minute, plug in and turn on).
