PLW1000v2 has no connectivity. Pick A Plug LED won't stay on.

I have 1 X PLW1000v2 and 3 X PL1000v2 which have been functional for a year or so. Recently the wireless stopped working on the PLW1000v2 and upon further inspection the Pick A Plug light will not stay on so there is no connectivity to the router. I see a "Failed to obtain IP address" from any client connected to the  PLW1000v2 which tells me the wireless is working on the PLW1000v2 but it is not connected to the PL1000v2 adapter plugged into the router. The other PL1000v2 adapters are working fine connected with Ethernet cables so it is only an issue with the PLW (wireless adapter). I have reset all to factory settings many times but get same result. I have moved the PLW to the same room as the router with same result. Below is a timeline of LED status light changes when plugging in the PLW device using the manual designated LED light #'s from left to right (7 3 1 2) as shown below. At 58 seconds, the Pick A Plug LED goes out and stays out.
 
Does anyone have ideas on how to fix or troubleshoot this?

5 - 00:42: 7 off, 3 ON, 1 ON, 2 off  (Pick A Plug LED reconnects)
6 - 00:54: 7 ON, 3 ON, 1 ON, 2 off
7 - 00:58: 7 ON, 3 off, 1 ON, 2  off  (Pick A Plug LED disconnects for good)
