new extender setup form

When I go to this website http://www.mywifiext.net/ I see NEW EXTENDER SETUP...When complete the new extender setup nothing happens when I click next

    When I select NEW EXTENDER SETUP I go to a paper where I have to fill out a form (Name, Password, Security questions). After I complete this from I select NEXT...At this point, nothing happens...The NEXT button is green, but nothing happens when I select it.
